Procedure for Providing Assistive Devices

The Ministry of Labor and Welfare continues to provide assistive devices based on state certificates.

Persons with disabilities, as well as individuals with other social statuses, may apply for a state certificate at the territorial center of the Unified Social Service (USS) serving their place of residence. Applicants must submit the required documents, based on which they will be issued a state certificate and a list of organizations from which they can obtain the assistive device they need.

To obtain a state certificate for wheelchairs, walkers, crutches, canes, upper- and lower-limb prostheses, hearing aids, and other assistive devices, applicants must submit the following documents:

  • Application,
  • Identity document,
  • Social security card, public service number or a certificate confirming that no public service number has been obtained,
  • If applying through a representative, a written power of attorney issued by the beneficiary and the representative’s passport.

Persons with disabilities must additionally submit a medical and social assessment decision (disability certificate) and an individual rehabilitation plan issued by the medical and social assessment commission responsible for determining disability.

For hearing aids, applicants must additionally submit a certificate issued by an ear, nose, and throat (ENT) specialist confirming hearing loss and the need for a hearing aid, as well as an audiometry report.

For orthoses, in addition to the above-mentioned documents, applicants under the age of 18 must submit a conclusion issued by a specialized medical organization, regardless of whether they have a disability. Applicants aged 18 and over must submit a conclusion issued by the Unified Social Service if they have a disability.

Socially disadvantaged persons must additionally submit a medical certificate confirming the need for the assistive device, as well as a certificate confirming their socially disadvantaged status.

To obtain a state certificate, the beneficiary or their representative, or the legal representative of a child under the age of 14 or a person under guardianship (parent, adoptive parent, or guardian), may submit an application in person at a territorial center of the Unified Social Service or online, in accordance with the procedure established by the legislation of the Republic of Armenia, using an electronic signature. If a person has been declared legally incapacitated, their guardian must submit the application for the provision of assistive devices.

In cases of severe mobility impairments where a person is unable to visit a territorial center of the Unified Social Service in person, they may contact the Service by telephone to request that an application for a state certificate be processed at their home or in a hospital. Within three working days of receiving the call, a visit will be arranged to the beneficiary’s location to facilitate completion of the relevant application.

For disputed cases concerning assistive devices covered by a warranty, as well as requests concerning repair services, the type of repair required, or repair services before the end of the established service period; beneficiaries may contact the Medical and Social Assessment and Assistive Devices Provision Department of the Unified Social Service (129 Armenakyan Street, Yerevan).
