The main objective of the “Home Care Service for Children with Disabilities” measure is to provide care to children with disabilities in their home environment, promoting their right to live in a family setting and preventing their admission to residential care institutions.

The following services are provided to beneficiaries in their home environment:

• assessment of the individual social needs of the beneficiary applying for care services and conclusion of an agreement for the provision of services corresponding to the assessed social needs,
• development of an individual social plan,
• provision of care services in accordance with the agreement concluded with the beneficiary, the service delivery schedule, and the individual social plan, including:
1. household assistance,
2. medical care and services,
3. social and psychological support,
4. counselling services;
• other services specified in the agreement.
