Day Care Services for Older Persons and Persons with Disabilities
Day Care Services (including Meals) Include:
- assessment of the individual social needs of day care center beneficiaries,
- development of an individual social plan based on the assessed needs,
- organization of leisure activities and engagement opportunities according to the beneficiary’s preferences,
- provision of one meal per day (5 days a week),
- organization of cultural events, meetings, excursions, and various clubs,
- provision of other services prescribed by law.
The services are provided with co-financing from the Ministry of Labor and Social Affairs through contracting non-governmental organizations.
To benefit from the above-mentioned services, citizens may apply to the territorial center of the Unified Social Service serving their actual place of residence, where an assessment of the person’s needs is conducted and a conclusion is issued regarding whether care is required. In case of a positive conclusion, the person is referred to an organization providing day care services.
The following documents are required to benefit from the Program:
- An identity document, social security card or Public Services Number (PSN), or a certificate confirming that no Public Services Number has been assigned.
- In the case of a foreign citizen or stateless person holding a residence permit in the Republic of Armenia. An identity document, a temporary or permanent residence card confirming the right to reside in the Republic of Armenia, or, in the case of an asylum seeker, an asylum seeker identification certificate, in the case of a person with refugee status, a travel document.
- In the case of a person with a disability, a decision or certificate extract issued because of a medical-social examination or functional assessment of the person.
- A medical card in the form approved by Joint Order No. 69-N of November 26, 2015, of the Minister of Labor and Social Affairs of the Republic of Armenia and Order No. 55-N of December 2, 2015, of the Minister of Health of the Republic of Armenia, issued by the relevant medical institution(s).
- Male applicants: a military service record book or a relevant certificate issued by the military registration and enlistment office.
To receive specialized care, in addition to the documents listed above, the person must also submit:
- An extract from the person’s medical history or from the outpatient medical record of a person with a mental health or intellectual disability who is under continuous supervision at a psychiatric organization providing outpatient psychiatric care and services.
- If the application is submitted by the person’s guardian: a copy of the court decision declaring the person legally incapacitated, a copy of the decision of the guardianship and trusteeship authority establishing guardianship over the person, and the guardian’s identity document.
