Home Care Services for Older Persons and Persons with Disabilities Service Description
For home-based care, beneficiaries receive the following social services based on their assessed social needs and health condition:
- Domestic assistance.
- Medical care and services.
- Social and psychological support.
- Counseling.
- Assistance in arranging the provision of assistive devices or access to other services.
Based on the beneficiary’s assessed needs, the following additional services may also be provided at home:
- Counseling for beneficiaries to support the restoration of their mental health, as well as counseling for their families on the specific needs of persons with mental health and other behavioral disorders, communication skills, and social rights.
- Development of independent living skills, including self-care, household management, mobility, communication, learning, and the ability to establish interpersonal and social relationships.
- Regular monitoring by a psychiatrist.
Depending on their health condition and the degree of limitation in their activities of daily living or functional capacity, persons receiving home-based care are classified into two groups. The authorized state body establishes the descriptions of these groups, the care delivery arrangements, and the corresponding staffing positions.
Who Can Apply?
Home care services are available to older persons and persons with disabilities who require such care and unemployed, except for those working from home.
Application Procedure
Applications, registration on the waiting list, and referrals for care services are processed through the Information Subsystem for Care Service Applications, Waiting List Management, and Referrals. Care services are provided based on an application submitted for this purpose. The applicant may be the person seeking care or their legal representative.
To apply for care services, applicants must access the e-social.am platform, select “Overcoming a Difficult Life Situation,” and then proceed to “Care Services Waiting List Registration.”
If the applicant does not have a personal account, they must register or verify their identity through the “Yes, I Am” national identification platform. They can then complete the care services waiting list application by selecting the relevant type of care and attaching scanned copies of the required documents.
Within 10 working days of confirmation of the application, a social worker from the territorial center of the Unified Social Service conducts a home visit to assess the applicant’s needs. Based on the assessment, a decision is made on whether to provide or deny the care service.
If the decision is positive, the applicant is automatically added to the relevant waiting list. When a place becomes available, the system automatically generates a referral, which is sent to the applicant’s personal account and email address.
Required Documents
Following submission of an online application for care services, the applicant’s data are retrieved from or exchanged with the relevant information databases. Where possible, the required information is obtained automatically through interoperability between information systems.
If the required data cannot be retrieved automatically from state or other information databases, the applicant must upload scanned copies of the following documents, as applicable:
- If the care seeker’s legal representative submits the application: a scanned copy of the document confirming their status as a legal representative, including a power of attorney, which does not require notarization.
- If the care seeker’s guardian submits the application: scanned copies of the court decision declaring the care seeker legally incapacitated and the decision of the guardianship and trusteeship authority establishing guardianship.
- An extract from the outpatient medical record.
